INTRODUCTION: The prevalence of polycystic ovarian syndrome (PCOS) in adolescent girls is between 1 and 4.3%. It remains controversial whether women with a history of idiopathic central precocious puberty (ICPP) are at increased risk for PCOS. Our objective was to assess the prevalence of PCOS in adolescents with a history of ICPP compared with healthy adolescents and the prevalence of PCOS among ICPP girls who have received or not gonadotropin-releasing hormone analogue (GnRHa) treatment. METHODS: We assessed post-menarcheal girls with a history of ICPP. Girls were evaluated at gynecological age ≥2.5 years. Data collected were age at menarche, menstrual cycle characteristics, BMI, clinical hyperandrogenism (HA), total and free testosterone levels. PCOS diagnosis was defined by criteria for adolescents. Subjects were also analyzed regarding whether or not they had received GnRHa treatment. RESULTS: Ninety-four subjects were assessed, and 63 had been treated with GnRHa. Menstrual disorders were found in 29%, clinical HA in 36%, and biochemical HA in 23%. Twelve percent met the diagnostic criteria for PCOS. There was no difference in BMI or in the incidence of menstrual dysfunction or hyperandrogenemia between treated and untreated patients. A higher proportion of clinical HA was found in untreated patients when compared to treated girls. The relative risk (RR) of developing PCOS in ICPP girls was 2.5 compared to a population of healthy adolescents. This RR was not higher in patients who received treatment with GnRHa than in those who did not. CONCLUSION: Adolescent girls with a history of ICPP have an increased risk of PCOS. This risk seems not to be related to GnRHa treatment.

La pubertad se manifiesta inicialmente por la aparición de los caracteres sexuales secundarios, como consecuencia de cambios hormonales que progresivamente conducen a la madurez sexual completa. En Argentina y el mundo, la pandemia ocasionada por el coronavirus SARS-CoV-2 generó un confinamiento que pudo haber interferido en el inicio y tempo del desarrollo puberal. Objetivo. Describir la percepción de los endocrinólogos pediatras del país sobre las consultas por sospecha de pubertad precoz y/o pubertad de rápida progresión durante la pandemia. Materiales y métodos. Estudio descriptivo, observacional, transversal. Encuesta anónima a endocrinólogos pediatras pertenecientes a la Sociedad Argentina de Pediatría y/o a la Asociación de Endocrinología Pediátrica Argentina, en diciembre de 2021. Resultados. Respondieron la encuesta 83 de 144 endocrinólogos pediátricos (tasa de respuesta 58 %). Todos consideraron que aumentó la consulta por desarrollo precoz o temprano, ya sea en sus variantes telarca precoz (84 %), pubarca precoz (26 %) y/o pubertad precoz (95 %). El 99 % acuerda con que se ha dado en mayor medida en niñas. La totalidad de los encuestados también considera que aumentó el diagnóstico de pubertad precoz central. El 96,4 % considera que ha aumentado el número de pacientes tratados con análogos de GnRH. Conclusión. Nuestros resultados sobre la percepción de endocrinólogos pediatras coinciden con datos publicados en otras regiones sobre el aumento del diagnóstico de pubertad precoz durante la pandemia por COVID-19. Se reafirma la necesidad de generar registros nacionales de pubertad precoz central, difundir las evidencias para su detección y abordaje oportuno.

In total, 96.4% of respondents consider that the number of patients treated with GnRH analogs has increased. Conclusion. Our results about the perception of pediatric endocrinologists are consistent with data published in other regions on the increase in the diagnosis of precocious puberty during the COVID-19 pandemic. We underscore the need to develop national registries of central precocious puberty, and to disseminate the evidence for a timely detection and management

INTRODUCTION: McCune-Albright syndrome (MAS) is a genetic disorder defined by fibrous dysplasia of bone, café-au-lait skin spots, and autonomous hyperfunction of one or more endocrine organs. MAS is caused by activating mutations of the GNAS1 gene. The most frequent type of endocrinopathy is gonadal endocrinopathy in the form of peripheral precocious puberty. OBJECTIVE: To describe the clinical characteristics, laboratory and imaging tests at the time of diagnosis and over the course of the disease, focusing on the classical triad of MAS. POPULATION AND METHODS: Observational, descriptive, retrospective clinical study of patients with MAS seen at the Department of Endocrinology of Hospital de Niños Ricardo Gutiérrez between 1974 and 2019. RESULTS: Twelve girls are described, all of whom developed peripheral precocious puberty (PPP) secondary to functional ovarian cysts. Their age at presentation was early (2.6 ± 1.3 years). Gonadotropin levels were suppressed or in the prepubertal range with generally high estradiol levels. Ten girls had café-au-lait skin spots since birth. During the course of disease, polyostotic fibrous dysplasia was detected in all patients. The treatments used to reduce ovarian cyst recurrence and hyperestrogenism effects showed varied effectiveness. CONCLUSIONS: In this series, the onset of PPP helped to make an early diagnosis of MAS and was difficult to treat. The course of disease showed persistent gonadal hyperfunction and worsening of bone injuries.

El SMA es producido por mutaciones activadoras del gen GNAS1. La endocrinopatía más frecuente es la gonadal, que se manifiesta como pubertad precoz periférica. Objetivo. Los niveles de gonadotrofinas estuvieron suprimidos o en rango prepuberal con niveles de estradiol generalmente elevados. Diez niñas tuvieron manchas "café con leche" desde el nacimiento. Durante la evolución se detectó displasia fibrosa poliostótica en todas las pacientes. Los tratamientos utilizados para disminuir la recurrencia de los quistes ováricos y los efectos del hiperestrogenismo mostraron diferente eficacia. Conclusiones. En esta serie, la aparición de PPP contribuyó al diagnóstico temprano del SMA y fue de difícil tratamiento. En la evolución persistió la hiperfunción gonadal y empeoraron las lesiones óseas.

CONTEXT: Loss-of-function mutations of makorin RING finger protein 3 (MKRN3) are the most common monogenic cause of familial central precocious puberty (CPP). OBJECTIVE: To describe the clinical and hormonal features of a large cohort of patients with CPP due to MKRN3 mutations and compare the characteristics of different types of genetic defects. METHODS: Multiethnic cohort of 716 patients with familial or idiopathic CPP screened for MKRN3 mutations using Sanger sequencing. A group of 156 Brazilian girls with idiopathic CPP (ICPP) was used as control group. RESULTS: Seventy-one patients (45 girls and 26 boys from 36 families) had 18 different loss-of-function MKRN3 mutations. Eight mutations were classified as severe (70% of patients). Among the 71 patients, first pubertal signs occurred at 6.2 ±â€…1.2 years in girls and 7.1 ±â€…1.5 years in boys. Girls with MKRN3 mutations had a shorter delay between puberty onset and first evaluation and higher follicle-stimulating hormone levels than ICPP. Patients with severe MKRN3 mutations had a greater bone age advancement than patients with missense mutations (2.3 ±â€…1.6 vs 1.6 ±â€…1.4 years, P = .048), and had higher basal luteinizing hormone levels (2.2 ±â€…1.8 vs 1.1 ±â€…1.1 UI/L, P = .018) at the time of presentation. Computational protein modeling revealed that 60% of the missense mutations were predicted to cause protein destabilization. CONCLUSION: Inherited premature activation of the reproductive axis caused by loss-of-function mutations of MKRN3 is clinically indistinct from ICPP. However, the type of genetic defect may affect bone age maturation and gonadotropin levels.

CONTEXT: Gonadotropin-releasing hormone agonists (GnRHas) are standard of care for central precocious puberty (CPP). A 6-month subcutaneous injection has recently been approved by the Food and Drug Administration. OBJECTIVE: Determine efficacy, pharmacokinetics, and safety of 6-month 45-mg subcutaneous leuprolide acetate for CPP. DESIGN: Phase 3 multicenter, open-label, single-arm study. SETTING: 25 sites in 6 countries. SUBJECTS: 64 GnRHa-naïve children with CPP (age: 7.5 ± 0.1 years) received study drug: 59 completed the study. INTERVENTION(S): 2 doses of 45-mg subcutaneous leuprolide acetate (0.375 mL) at 0 and 24 weeks; children were followed for 48 weeks. MAIN OUTCOME MEASURE(S): Percentage of children with serum luteinizing hormone (LH) <4 IU/L 30 minutes following GnRHa stimulation at week 24. RESULTS: 54/62 (87%) children achieved poststimulation LH <4 IU/L at week 24; 49/56 (88%) girls and 1/2 boys maintained peak LH <4 IU/L at week 48. Mean growth velocity decreased from 8.9 cm/year at week 4 to 6.0 cm/year at week 48. Mean bone age was advanced 3.0 years beyond chronological age at screening and 2.7 years at week 48. Breast pubertal stage regressed or was stable in 97% of girls and external genitalia development regressed in both boys. Adverse events were mild and did not cause treatment discontinuation. CONCLUSIONS: A small volume of 45-mg subcutaneous leuprolide acetate administered at a 6-month interval effectively suppressed pubertal hormones and stopped or caused regression of pubertal progression. This long-acting GnRHa preparation of leuprolide acetate is a new, effective, and well-tolerated therapy for children with CPP.

Background Puberty is associated with a physiological decline in insulin sensitivity (IS). Overweight (OW) and obesity (OB) are common among girls with central precocious puberty (CPP). CPP is considered a risk factor for metabolic diseases. The aim of this study was to assess surrogate measures of IS, body mass index (BMI) and other metabolic parameters in CPP girls at diagnosis and during treatment with gonadotropin-releasing hormone analogues (GnRHa). Methods We present a prospective longitudinal study of CPP girls. The standard oral glucose tolerance test, homeostatic model assessment of insulin resistance (HOMA-IR), whole-body IS index (ISI) and fasting lipid profiles were evaluated at diagnosis, and at 6 and 12 months of treatment. Results Nineteen CPP girls were included; 17 were evaluable. At baseline, seven patients had normal weight (NW), five were OW and five were OB. During GnRHa treatment no significant changes were observed in BMI, HOMA-IR or ISI when considering the whole group. Whereas, when we analyzed patients according to BMI status, in NW patients, BMI increased significantly with no changes in HOMA-IR or ISI along treatment. In the OW/OB group, no significant differences were observed in BMI, HOMA-IR or ISI. Conclusions Girls with CPP showed a high frequency of OW/OB and a high prevalence of IR. GnRHa did not affect BMI, IS or the lipid profile when considering the whole cohort of patients. However, there was an increase in BMI in NW girls but not in OW/OB patients.

Se define como desarrollo sexual precoz en la niña a la aparición de caracteres sexuales secundarios antes de los 8 años. Existen distintos tipos de desarrollo sexual precoz: Pubertad Precoz Central (PPC) producida por la reactivación prematura del eje hipotálamo-hipófisogonadal (HHG), Pubertad Precoz Periférica producida por actividad ovárica autónoma independiente del eje HHG y variantes del desarrollo sexual (pubarca y telarca precoz aisladas). Recientemente los avances en estudios moleculares y por imágenes han permitido precisar mejor su etiología. El diagnóstico se basa en el examen físico, análisis de laboratorio y estudios por imágenes que permiten diferenciar las formas completas de sus variantes. La PPC produce alteraciones emocionales en las niñas y aceleran la maduración esquelética comprometiendo la talla adulta por lo cual es necesario instituir el tratamiento adecuado. La terapéutica de elección son los análogos de GnRH que mostraron ser seguros y efectivos en las niñas con PPC. Se presenta una revisión del tema enfatizando en las herramientas de utilidad para orientar al pediatra en el diagnóstico y realizar la pronta derivación al especialista en endocrinología infantil para el tratamiento y seguimiento de niñas con distintos tipos de desarrollo sexual precoz


The appearance of secondary sexual characteristics before the age of 8 in girls is defined as early sexual development. There are different types of early sexual development: Central Precocious Puberty (CPP) produced by the premature reactivation of the hypothalamic-pituitary-gonadal axis (HPG), Peripheral Precocious Puberty produced by autonomous ovarian activity independent of the HPG axis and variants of sexual development (premature pubarche and telarche). Recently, advances in molecular studies and imaging have allowed to better define the etiology of early sexual development. The diagnosis is based on physical examination, laboratory analysis and imaging studies that allow differentiation of the complete form from their variants. CPP produces emotional alterations in girls and accelerates skeletal maturation, compromising adult height. After confirming diagnosis it is necessary to institute the appropriate treatment. GnRH analogues have shown to be safe and effective in girls with CPP. A review of the topic is presented, emphasizing on the useful tools to guide the pediatrician in the diagnosis and prompt referral to a pediatric endocrinologist for the treatment and monitoring of girls with different types of early sexual development

El síndrome de poliquistosis ovárica (SOP) es un cuadro que acompaña a la mujer durante toda su vida y se caracteriza por hiperandrogenismo y anovulación crónica. Se presenta comúnmente en la adolescencia y es eldesorden endócrino más frecuente en mujeres en edad reproductiva en el mundo. A largo plazo se asocia con morbilidad significativa que incluye alteraciones en la salud reproductiva, disfunción psicosocial, síndrome metabólico, enfermedad cardiovascular e incremento en el riesgo de cáncer. Su etiología es desconocida aún, sin embargo, a lo largo de las tres últimas décadas, diferentes grupos de expertos en el mundo han elaborado guías para el diagnóstico y manejo de esta enfermedad. Existen tres grupos diferentes de criterios en el mundo pero están basados principalmente en información y experiencia en el manejo de mujeres adultas. Estos criterios diagnósticos no son completamente trasladables a las adolescentes. El objetivo de este artículo es acercar al pediatra clínico los elementos para alcanzar un entendimiento práctico y simplificar el manejo inicial del diagnóstico del cuadro de SOP en la adolescencia, concientizar acerca de las comorbilidades asociadas y su posibilidad de prevención para evitar riesgos en la vida adulta.


The polycystic ovarian syndrome (PCOS) is a lifelong disorder characterized by hyperandrogenism and chronicanovulation. It becomes manifest soon after puberty and it is the most frequent endocrine disorder in women at reproductive age in the world. In the long term it is associated with significant morbidity that includes alterations in reproductive health, psychosocial dysfunction, metabolic syndrome, cardiovascular disease and increased risk of cancer. Its etiology is still unknown, however, over the last three decades; several guidelines for the diagnosis and management of this disease have been developed. Three different sets of diagnostic criteria have been established to define this disease in adult women, but these diagnostic criteria are not completely transferable to adolescents. The objective of this article is to give pediatricians the elements for a practical understanding to simplify the initial management of the diagnosis of PCOS in adolescence and to raise awareness on the likelihood of associated comorbidities and that appropriate intervention could prevent later complications in adult life

El Síndrome de Turner es un desorden cromosómico causado por haploinsuficiencia completa o parcial de uno de los cromosomas sexuales. Incidencia 1: 2500 recién nacidas vivas. Clínicamente las pacientes presentan talla baja, un espectro amplio de anomalías somáticas y disgenesia gonadal. Desde el año 1968 hasta el presente se estudiaron clínica y citogenéticamente 630 niñas con fenotipo de Turner, sin ambigüedad genital y con cariotipos anormales, quienes consultaron en la División de Endocrinología del Hospital de Niños "Ricardo Gutiérrez". Se realizó cariotipo en sangre, al inicio con metodología estándar, luego con diferentes bandeos convencionales y de alta resolución. En casos especiales se aplicó la técnica FISH y el análisis molecular de los cromosomas X e Y. El número de metafases analizadas también varió con el tiempo, permitiendo evidenciar más de una línea celular. En casos de alta sospecha clínica, la lectura de 100 metafases permitió poner en evidencia mosaicismos bajos conteniendo la línea 45,X. En nuestra serie la monosomía de cromosoma sexual 45, X fue la más frecuente siguiendo los mosaicos numéricos y estructurales de uno de los cromosomas sexuales. Los diferentes hallazgos cromosómicos nos han permitido establecer una correlación fenotipo-cariotipo en regiones específicas de los cromosomas sexuales


Turner Syndrome is a common chromosomal disorder caused by total or partial haploinsufficiency of one of the sex chromosomes. Incidence: 1: 2500. Clinically is characterized by short stature, several typical somatic features, and gonadal dysgenesis. This is a retrospective study involving 630 girls with Turner phenotype and abnormal karyotype, evaluated at the Endocrinology Division of Children´s Hospital "Ricardo Gutiérrez" between 1968 and 2018. The karyotype was done in leucocytes from peripheral blood and the metaphases were analyzed at the beginning with standard methodology and then with different banding techniques, standard and high resolution. In special cases, the FISH technique and the molecular analysis of the X and Y chromosomes were applied. The number of metaphases analyzed also changed with time, allowing the finding of more than one cellular line. In cases where the clinical suspicion was strong, the analysis of 100 metaphases allowed us to put in evidence low mosaicisms containing the 45,X line. In our study the monosomy of sexual chromosome 45, X was the most frequent following the numerical and structural mosaics of one of the sex chromosomes. The different chromosomal constitutions have contributed to establish in our patients phenotype-karyotype correlation with specific regions of the sex chromosomes

BACKGROUND: Estradiol at baseline or after a classical gonadotropin-releasing hormone test did not reflect ovarian steroidogenesis in central precocious puberty (CPP) girls. AIMS: To evaluate estradiol response to depot triptorelin, both at start and during therapy to determine how active ovarian steroidogenesis is at pubertal stage and under therapy. METHODS: A prospective study was performed in 43 CPP girls. Serum luteinizing hormone and follicle-stimulating hormone at 3 h (LH-3h, FSH-3h) and estradiol at 24 h (E2-24h) after injection of depot triptorelin 3.75 mg were measured, at first dose and at 3, 6, 12, 18 and 24 months of treatment. RESULTS: E2-24h after depot triptorelin was >100 pg/ml after the first dose. Estradiol response (E2-24h) fell to levels <14 pg/ml in 78 out of 82 follow-up visits along 2 years of therapy. Concomitantly, LH-3h and FSH-3h were <4.0 and <6.3 IU/l, respectively. In 4 patients with inadequate treatment, E2-24h, LH-3h and FSH-3h rose to pubertal values similar to those observed at first dose. CONCLUSION: Estradiol (<14 pg/ml) assessment 24 h after depot triptorelin administration is a reliable and simple manner to confirm ovarian suppression in CPP girls during treatment.

CONTEXT: The GnRH test is the gold standard to confirm the diagnosis of central precocious puberty (CPP); however, this compound is not always readily available. Diagnostic accuracy of subcutaneous GnRH analogues tests compared to classical GnRH test has not been reported. OBJECTIVE: To evaluate the diagnostic accuracy of Triptorelin test (index test) compared to the GnRH test (reference test) in girls with suspicion of CPP. DESIGN: A prospective, case-control, randomized clinical trial was performed. CPP or precocious thelarche (PT) was diagnosed according to maximal LH response to GnRH test and clinical characteristics during follow-up. PATIENTS AND INTERVENTIONS: Forty-six girls with premature breast development randomly underwent two tests: (i) intravenous GnRH 100 µg, (ii) subcutaneous Triptorelin acetate (0.1 mg/m(2), to a maximum of 0.1 mg) with blood sampling at 0, 3 and 24 h for LH, FSH and estradiol ascertainment. MEASUREMENTS: Gonadotrophins and estradiol responses to Triptorelin test were measured by ultrasensitive assays. RESULTS: Clinical features were similar between CPP (n = 33) and PT (n = 13) groups. Using receiver operating characteristic curves, maximal LH response (LH-3 h) under Triptorelin test ≥ 7 IU/l by immunofluorometric assay (IFMA) or ≥ 8 IU/l by electrochemiluminescence immunoassay (ECLIA) confirmed the diagnosis of CPP with specificity of 1.00 (95% CI: 0.75-1.00) and sensitivity 0.76 (95% CI: 0.58-0.89). Considering either LH-3 h or maximal estradiol response at 24 h (cut-off value, 295 pm), maintaining the specificity at 1.00, the test sensitivity increased to 0.94 (95% CI: 0.80-0.99) and the diagnostic efficiency to 96%. CONCLUSION: The Triptorelin test had high accuracy for the differential diagnosis of CPP vs PT in girls providing a valid alternative to the classical GnRH test. This test also allowed a comprehensive evaluation of the pituitary-ovarian axis.

OBJECTIVE: To report genotype-phenotype correlation in a large cohort of patients. CONTEXT: Study of the CYP21A2 gene in 866 unrelated chromosomes of 21-hydroxylase deficiency in Argentinean patients with classic and nonclassic (NC) forms of congenital adrenal hyperplasia (CAH). METHODS: Eleven most common mutations were analysed by allele-specific polymerase chain reaction, restriction fragment length polymorphism (RFLP) or southern blot analysis. Gene sequencing was performed when no mutation was detected in one allele or the genotype-phenotype correlation was lacking. RESULTS: The 11-most-common-mutation screening allowed for the detection of 88·1% of affected alleles (80·3% in the NC and 95·2% in the classic forms). p.V281L, IVS2-13A/C>G (In2) and gene deletions and large gene conversions were the most prevalent mutations. In2 (35·2%) in salt wasting (SW), p.I172N (37·3%) in simple virilizing and p.V281L (54·1%) in NC CAH were the most prevalent mutations within the clinical forms. In 7/15 p.P30L mutation alleles, a chimeric CYP21A1P/CYP21A2 gene [PromCYP21A1P; p.P30L] was detected, while 6/15 represented a single-nucleotide substitution, and in 2/15 linkage with mutations, p.[P30L; V281L] and [p.P30L; IVS2-13A/C > G; p.Q318X] was found. In two SW patients, a novel nonsense mutation, p.Q41X, was observed. In three p.V281L mutation patients, the phenotype was more severe than predicted by genotype. Sequence analysis revealed an intronic alteration in the allele carrying the p.V281L mutation [IVS2 + 5G > A; p.V281L]. An aberrant splicing in this p.V281L mutated allele explains the clinical phenotype. CONCLUSIONS: A high percentage of CYP21A2 affected alleles is detected by the 11-mutation screening study. Genotype-phenotype correlation was high, but when the phenotype is more severe than predicted by genotype, presence of two alterations in one allele should be ruled out.

UNLABELLED: Gonadotroph adenomas are difficult to diagnose since they usually show as nonsecreting tumors or produce biologically inactive hormones with no clinical effects and classically grow silent until neurological symptoms appear. Presentation with bilateral ovarian masses and ovarian hyperstimulation has been described in fertile years. Gonadotroph adenomas are extremely infrequent in children. We report a 13-year-old postmenarcheal girl referred to our hospital with 6 months of amenorrhea, abdominal palpable mass presumptive of bilateral ovarian tumors. The patient had Tanner IV breast development and a large abdominal mass occupying the whole low hemiabdomen. Laboratory evaluation revealed high estradiol levels with suppressed luteinizing hormone and inappropriately high follicle-stimulating hormone (FSH) levels. Pelvic ultrasound showed enlarged ovaries containing multiple giant cysts. An MRI revealed a pituitary macroadenoma. Transsphenoidal resection of the adenoma was performed with an uneventful postoperative course. Immunohistologic examination only showed staining for FSH, thus confirming pituitary secreting FSH adenoma. Hormonal laboratory levels normalized and ovarian masses showed marked involution 1 month after surgery. Three months later the MRI showed tumor disappearance. CONCLUSION: The presence of bilateral ovarian tumors requires a careful endocrine and neurological evaluation to exclude the presence of an FSH-producing tumor in order to avoid unnecessary ovarian surgery.

UNLABELLED: Some adolescents with a history of idiopathic central precocious puberty (ICPP) develop hyperandrogenism. HYPOTHESIS: Luteinizing hormone (LH) hypersecretion could be a common mechanism underlying ICPP and polycystic ovary syndrome. AIM: To explore the GnRH-LH axis in those patients. DESIGN: To compare overnight LH secretion in 7 healthy adolescents (CG) with that in patients with prior ICPP [5 with (CPPA) and 7 without (CPPB) hyperandrogenism]. To analyze daytime LH secretion in those patients. METHODS: LH secretion was quantified by immunofluorometry and deconvolution analysis. RESULTS: Nighttime mean LH (international units/liter) was higher in CPPA (6.9 +/- 1.5) than in CPPB (3.2 +/- 0.4, p < 0.05) and CG (2.9 +/- 0.4, p < 0.01). Deconvolution analysis revealed a greater nighttime LH frequency (pulses/hour) both in CPPA (0.91 +/- 0.06, p < 0.01) and CPPB (0.74 +/- 0.02, p < 0.05) than in CG (0.45 +/- 0.07). CPPA patients maintained a higher frequency than CPPB. Pulsatile LH production was greater in CPPA than in CG (50 +/- 12 vs. 18 +/- 3 IU/l/day, p < 0.01). Daytime mass of LH released per burst and pulsatile production rate were significantly greater in CPPA than in CPPB patients. CONCLUSIONS: Hyperandrogenic adolescents with prior ICPP show increased pulsatile LH secretion. Augmentation of LH pulsatility may predispose to or cause hyperandrogenism in some adolescents with a history of precocious puberty.

RESUMEN. Introducción. El síndrome de Rokitansky es unaanomalía grave del tracto reproductivo femenino,con ausencia de menarca e imposibilidad deembarazo.Objetivo. Analizar las características clínicas, diagnósticas,terapéuticas y evolutivas de un grupo deadolescentes afectadas.Material y métodos. Nueve adolescentes (15,2 ± 1,5años) que consultaron por ausencia de menarca odolor pelviano se evaluaron mediante examen físico,ecografía ginecológica, resonancia nuclear magnéticaabdominopelviana, cariotipo, dosajes hormonalesy laparoscopia. El diagnóstico final fue desíndrome de RokitanskyResultados. Todas las pacientes tuvieron caracteressexuales secundarios normales. Tres presentaronanomalías renales y tres anomalías esqueléticas.La ecografía mostró útero ausente, rudimentarioo asimétrico, este último con hematómetra evidenciadopor resonancia nuclear magnética. Lalaparoscopia detectó anomalías tubarias u ováricasen seis casos. Mediante estos estudios, cinco denuestras pacientes se clasificaron como pertenecientesa la forma B (atípica) del síndrome. Dospacientes tuvieron cariotipos anormales: 46, X, del(X) (q22q24) y 46,XX, t (3;22) (q13.3, q 12.1), ambassin insuficiencia ovárica. Seis pacientes tuvieronconcentraciones hormonales normales, una presentóhiperplasia suprarrenal congénita de formano clásica, confirmada por estudio molecular y otramostró alteraciones compatibles con síndrome depoliquistosis ovárica. En seis pacientes se efectuóplástica de vagina con resultado satisfactorio
